Beginning of the Year Parent Technology Survey

While preparing for the start of the school year, I stumbled upon this parent survey about technology:

I tweaked it a little for my grade level purposes:

But I still think it's an awesome way to figure out what students have at home.  I sent it to my administration for feedback and approval, then I'll check with my grade level, have it translated into Spanish and send home on the first day!
